Looking at your Blogger profile I think it is really well rounded, and don't need improvements at all. I was wondering where did you get background at? I was looking for a kid/school related background. I Like you profile though! Its Cute! Lol. :)
This is a great form! I especially like the question about how their child will be getting home from school. This is a very important thing to know and I would be very impressed as a parent if you ask me this.
That is awesome! My daughter's teacher made me fill a survey out so she will know a little more about my daughter, and us(Parents). :)
